Player & Volunteer Registration Instructions
Fill out Player and Volunteer Applications online at eAYSO. You can use the "eAYSO Web Site" link above which will open in a separate window.
Please read all of the information below before starting!

Note: Submitting an on-line player Application does NOT mean
that your child is registered in AYSO Region 9. Your child is registered only AFTER our Registrar has received your signed
forms, payment and validated your information for completeness.
The eAYSO website may not work on all browsers. We know that it works on the following, but it may work on others also:
- Internet Explorer 6.0 or higher
- Netscape 6.2 or higher
- Firefox 1.0 and higher
Allow pop-ups for this website.
Internet Explorer users: Click on Tools –> Pop-up Blocker –> Pop-up Blocker Settings. On the area “Address of
website to allow”, enter www.eayso.org and click on Add
Firefox users: Add the site after clicking on Tools –> Options –> Content -> Allowed sites
Your Internet Service Provider (ISP) may have its own pop-up blocker as well. Please turn this one off too.
Detailed instructions for using eAYSO can be found by clicking on Help in the upper right portion of most eAYSO screens. You will need
Adobe Reader to read the My eAYSO User Guide.
For Both Player and Volunteer Applications:
- Have you used eAYSO in the past? Go to the eAYSO web site and click on Log In. Reminder: your eAYSO username is typically
your e-mail address. If you can't remember your password from last year, try the Forgot Password option.
- Are you new to eAYSO? Select First Time and the next several screens will give you some basic information about using
the eAYSO website. Also, please note the following:
- You only need to create an eAYSO User account once! You can then submit Player Applications for multiple children in your
family and Volunteer Applications for you, your spouse/significant other and your older children (if they wish to be a Youth Referee), all
with this single login.
- It is possible to save 30 seconds and skip the eAYSO User account setup step, but if you do then you will
NOT be able to come back later and retrieve all of the Player or Volunteer information you spent so much time entering!
- When you get to the My eAYSO screen (screen 1), do one of the following:
On-line Player Application Instructions:
- Select a Region. Thousand Oaks is Region 9.
- Enter information about you and your child(ren). Please note the following:
- Any requested information marked with a red star * is required; you cannot continue with an
Application until all such fields have valid information.
- Please be sure to include a current, valid e-mail address for each parent: we rely heavily on electronic
communications!
- Every team in AYSO Region 9 must have 1 Coach, 1 Assistant Coach, at least 1 Team Manager and at least 1 Team Referee (except for
the U6 division). Even if you know nothing about the game of soccer, if you volunteer to coach or referee then Region 9 will teach you
everything you need to know - FOR FREE! Be sure to indicate on your child's registration which job (or jobs) you would
be willing to volunteer for. (Note: for referees we also supply a standard uniform except for shoes and watch.)
- Print out, date and sign one (1) copy of each Player Application and bring them to one of the two Walk-In Registrations.
You might want to print out an extra copy for your records. If your child is new to Region 9 then you will also
need to bring a copy of their Birth Certificate.
- Pay the Registration Fee at the Walk-In Registration (cash, check or credit card).
- After you have come to a Walk-In Registration, turned in your forms and paid the registration fees, Region 9 must still validate
and accept the registration. We try to accept every child from these two registrations, however, we may refuse or cancel an
Application under certain circumstances, (e.g. we have too many or too few players in an age group, not enough volunteers or not
enough field space). Your child is NOT registered with Region 9 until he or she is accepted by our Registrar.
You will be notified by your child's coach, a Division Administrator or the Registrar once they are accepted.
Please note that due to our large size, this may not be until mid-July.
On-line Volunteer Application Instructions:
- Select a Region. Thousand Oaks is Region 9.
- Enter information about yourself. Please note the following:
- Any requested information marked with a red star * is required; you cannot continue with an
Application until all such fields have valid information.
- Please be sure to include a current, valid e-mail address: we rely heavily on electronic communications!
- When entering your SSN, please be sure it is valid and accurate (123-45-6789 will NOT work!!). Otherwise,
the AYSO National Office will kick out the registration as invalid.
- When filling out a Youth Volunteer application, you must include parent/guardian information and their signature.
- If a Youth Volunteer is going to turn 18 during the season then they should submit an Adult Registration form as soon as is
practical after their 18th birthday.
- Print out, date and sign two (2) copies of the Volunteer Application and bring them to one of the two Walk-In Registrations.
You might want to print out an extra copy for your records. For new Region 9 Volunteers, we will need to
verify your identity with a current Drivers License or other valid government picture ID.
- After you apply, Region 9 must still accept your Volunteer registration. You are NOT registered as a Volunteer with Region 9
until you have been accepted by our CVPA.
